Choose general inquiry when your purpose is a general question. Use the separate cost, case-review or appointment path when that is the purpose of your request.

Select the patient profile and Hospital destinations, add a title and message, and include allowed optional attachments if relevant. A basic incomplete patient profile may be sufficient. Sign-in is required to submit.

Passport completion and medical reports are not requirements of the Hospital general-inquiry type. Sending sensitive patient information through another action still follows that action’s verification and sharing rules.
