Select the patient profile and Hospital destinations, add a title and cost question, and choose reports if relevant. A basic inquiry not tied to a clinical case may proceed without a completed passport, subject to the applicable policy.

Profile and document requirements come from the active Hospital request policy. Complete any missing requirements shown for your request. Each Hospital receives only its own request data and selected files.
