The request can include the condition, current symptoms, previous diagnoses and previous procedures or treatments when available, plus an additional summary when needed. Describe the information you have.

Select the patient profile and participating Hospitals. This path requires the identity and medical information set by Hospital policy. Choose the relevant reports and exact file versions to send.

Missing profile, verification or document requirements must be completed before submission. If a current passport is required and has not reached a Hospital, its first delivery needs a separate decision for that Hospital.

Each selected Hospital handles its own case and internal routing. ShifaTH coordinates the request and conversation; the Provider makes clinical decisions. Submission does not guarantee acceptance, a treatment plan or a clinical outcome.
