Preparing information for a Provider medical case review request

Organize the question and existing information for the receiving Provider. The Provider decides the clinical assessment and response it can offer.

In this guide

Clarify the question

Write what you want the Provider to address and what remains unclear after any previous professional discussion. Ask whether its case-review process fits that question. Do not assume that sending reports guarantees a specialist response or a formal second opinion.

Put existing information in order

List existing reports and relevant prior care by source and date where known. Keep originals available. Mark gaps or uncertain dates instead of creating a new diagnosis or filling in findings yourself.

Separate your questions from professional findings

Keep your goals and questions in a separate note. Identify a previous diagnosis or finding as something recorded by its professional source, using the original document when available. Ask the receiving Provider to explain medical meaning rather than interpreting a test or image in your preparation note.

Ask about language, format and requirements

Ask which existing records are relevant, which languages and formats can be used, whether a translation is needed and whether further information is required. These questions do not establish a universal file list or tell you to obtain a new test.

Clarify the response you can expect

Ask what kind of assessment or response the Provider can offer, what remains to be assessed in person and which questions cannot be answered from the submitted information. Ask the Provider to clarify its process rather than assuming telemedicine or treatment approval.

Keep clinical decisions with the Provider

Return questions about treatment, suitability, medical preparation and appointment decisions to the Provider or treating clinician. This guide organizes an administrative handoff; it does not diagnose, select treatment or interpret laboratory or imaging results.

Using ShifaTH

Medical case review is one of the Hospital request types. The receiving Hospital routes the case internally. Identity and report requirements follow the applicable rules. Selected documents are delivered to the chosen Organization; uploading a report privately does not deliver it. For exact requirements and sharing steps, use the canonical request and document explanations and authorized Help.
