Start with the patient’s preferences
Ask what the patient wants help with and who they want involved in communication. Record the questions the patient wants answered separately from your own practical questions. Avoid assuming that accompanying the patient means speaking for them in every decision.
Ask about permission for the actual task
Ask the Provider what permission or evidence it needs for communicating with you, receiving information or making the specific arrangement. A family or friendship relationship is not an answer to every authority question. Questions about legal representation, capacity or guardianship need the applicable approved legal guidance.
Organize records and practical questions
Identify existing records relevant to the question and ask the Provider what it needs. Make a separate list for timing, location, accommodation, transport and language questions. Assign each question to the person or organization that can answer it.
Agree how the patient will participate
Ask how the patient wishes to join discussions and receive updates. Clarify which information should be checked with the patient and which needs a clinician’s explanation. Keep unclear medical meaning as a question for the Provider rather than resolving it yourself.
Using ShifaTH: management, ownership and delegation
A User can manage a patient profile for another person within authorized access. The patient profile is separate from the sign-in account. Creating a managed profile does not by itself require a patient selfie; a verified patient claim is a different process. A successful claim keeps the existing profile and history. Delegated access grants only its selected permissions; a relationship label does not establish legal authority.
Prepare cross-language questions
Ask which language support the Provider can arrange for the actual interaction. Keep original records and prepare clear questions. Check with the Provider who should clarify medically significant wording; do not assume a relative or machine translation replaces that explanation.
Use the right explanation for the next step
Use patient-management and delegation explanations for the platform relationship, authorized Help for a profile-specific task, and the Provider for the actual care arrangement. Keep unresolved legal authority questions separate from the practical preparation checklist.
