Separate the decisions
Make three lists: questions for the healthcare Provider, questions for travel and accommodation providers, and questions for your treating clinician. Beside each item, note the person or organization responsible and whether you have an answer. A response about healthcare does not settle entry, transport or insurance questions.
Confirm the appointment and instructions
Ask the Provider: Has an appointment been confirmed, or are we still discussing a request? What date, local time, time zone and location apply? Who should confirm a change? Which instructions should come from the Provider before the actual visit? Keep the answers together rather than treating a preferred date as a booking.
Prepare existing information and language questions
List the records you already have and ask which are relevant. Keep readable originals and note their language. Ask what language support is available for the planned visit and whether any document translation is needed. Do not assume that a translated website means an interpreter will be available.
Ask the practical travel questions
Ask the relevant transport and accommodation providers about location, access, changes and cancellation conditions. Ask your insurer what it can confirm for this trip and this care. Record the answer’s scope and any unresolved question; do not assume that a Provider reply includes transport, accommodation or insurance arrangements.
Check official entry information
Check current official information for your nationality, residence and entry plans. Treat visa information and arrival-card information as separate questions. The Thai Ministry of Foreign Affairs’ e-Visa information and Thailand Immigration Bureau’s arrival-card information are source starting points; confirm which authority applies to your situation. This checklist does not determine entry eligibility.
Discuss travel and follow-up with your clinicians
Ask your treating clinician what needs to be considered before travel and who will handle follow-up. Ask the receiving Provider what information should be available for the next treating professional. Use their instructions for your actual care; this guide supplies no travel-clearance, medication or recovery timetable.
Recheck before departure
Recheck the appointment details, Provider instructions, existing records, language arrangements and official entry information. Mark unanswered items and return each one to its responsible source. Continue with the records or appointment-preparation guide for a more focused checklist.
Using ShifaTH
You can browse eligible public Provider information before signing in. An appointment request or preferred time is separate from the official appointment. ShifaTH is an access and coordination platform; the Provider remains responsible for clinical decisions and care.
