Create or select your own or a managed patient profile

Choose the patient before starting a request. A patient profile is separate from your sign-in account.

01

Before you start

Sign in to create or manage a profile. Know whether the profile is for you or someone else.

02

What to do

  1. Open patient-profile selection or management.
  2. Select an existing profile, or create your own or another person’s supported profile.
  3. Enter the required identifying information and relationship when applicable.
  4. Check completeness and use that profile for the intended request.
03

Optional medical information in the profile

Where available, keep a current medical summary and separate medical items such as allergies, conditions or medications. Add or update the relevant item, or end or archive it when appropriate. A supporting document can be referenced. Summary and items are not sent automatically; a request records the selected information shared at that time.

04

Expected result

One account can manage several profiles, each with its own history. An incomplete profile can be saved. A managed patient profile can exist before that patient creates their own User account.

05

If something is blocked or different

If an existing active patient profile is found, use the approved claim or linking path instead of a duplicate. Close family and Friend are supported relationships, but a label does not prove legal authority.
